Pet owner's address

Pet owner's primary (best) phone number

Pet owner's email address (mandatory)

Pet name

Pet type (Dog, Cat, Other)

Pet Breed

Pet sex

Pet weight

Pet Microchip # (a 9, 10 or 15 digit number)

Pet primary/secondary colors

Any pet medical or behavioral condition and medications if necessary

Veterinarian's name, Address and Phone # (optional, but better to have)

Emergency Contact Name and Phone # (optional, but good to have in case you can't be contacted)

Have a clear photo of your pet to upload

Have a valid credit card or payment method ready

🐾 Registering More Than One Pet?

Here's How to Use a Discount Code when using the Form to register:

Choose the number of pets you're registering

Use the Qty dropdown to select how many pets you'd like to register.

Enter your discount code

Type one of the following codes into the “Add promotion code” box at checkout: Register2 – for 2 pets

Register3 -- for 3 pets

Register4 -- for 4 pets

Register5 -- for 5 pets

Click "Apply" to see your savings The discount will be instantly subtracted from your total.

When using the Phone to register, the payment link will be texted to you to pay for up to 2 pets. If you have more than 2 pets to register, please use the online form.
